What Happens Next?

For the pharmacist to assess your suitability for treatment under the Common Conditions Service, we will require you to provide us with some further information. Please follow the directions below to submit this information and to choose your preferred pharmacy.

Once submitted, the pharmacist will review your information and a member of the pharmacy team will contact you by phone. As a result of the information provided by you, the pharmacist may arrange for a consultation with you at the pharmacy, offer advice or refer you to another appropriate healthcare professional if the Common Conditions Service is not suitable for you.
